Who this is for

If you’re finding day-to-day activities difficult, we can help you understand your options, check if you’re eligible for support, and make a referral. Start here to learn what to try first, what to expect, and how to make a request.

Adults (18+) who may need care and support because of illness, disability, or changes in circumstances. You can use this for yourself or on behalf of someone else.

To report an urgent concern (safeguarding)

  • Immediate risk of harm: Call 999.
  • Adult safeguarding concern call 0345 604 2719 (out of hours 0345 604 2886).

Providers: For non-urgent referrals use the Provider Referral Form (not for urgent or safeguarding). For urgent referrals, call 0300 111 8010 (out of hours 0345 604 2886).
