How to book an appointment
Registration must take place in the district where the death has occurred.
If you are unable to attend an office in Staffordshire, you can visit your local registration office and complete a death declaration appointment. Following your appointment, the information is shared via email with us, for registration to be completed, and all documentation including certificates will be posted to you.
