Referral Pathways

  • Start with the Assess, Plan, Do, Review cycle in your setting.
  • If concerns persist, discuss next steps with parents and carers.
  • Identify the most appropriate service based on the child’s needs (e.g., speech and language therapy, occupational therapy, specialist SEND support).
  • Follow local referral procedures and provide detailed information about the child’s development and interventions already tried.
  • Keep parents informed throughout the process and obtain consent before making referrals.

Guidance for Using Outside Agencies

  • Work collaboratively – share observations, assessments, and plans with professionals.
  • Attend meetings and contribute to multi-agency discussions.
  • Implement strategies recommended by specialists and monitor progress.
  • Maintain clear records of advice and actions taken.
  • Review support regularly and adapt as needed.
