The Enhanced Assess, Plan, Do, Review (EAPDR) pathway helps early years settings in Staffordshire provide additional support for children with special educational needs and disabilities (SEND).

This guidance explains how to complete the EAPDR request form correctly.
Before you start
- Make sure you have discussed the child’s needs with their parents or carers.
- Gather evidence of the child’s progress and any interventions already in place.
- Check that the child meets the criteria for the EAPDR pathway.
What you'll need
- Details about the child’s development and current support.
- Information from professionals involved with the child (if applicable).
- Consent from parents or carers to share information.
How to complete the form
- Child's details - include full name, date of birth, and setting information.
- Reason for request - explain why additional support is needed. Be clear and specific.
- Evidence of support - provide details of strategies already used and their impact.
- Professional impact - add any reports or advice from health or education professionals
- Parent/carer consent - confirm that consent has been obtained
Submitting the form
- Send the completed form to the Early Years SEND team within six weeks of identifying the need.
- Keep a copy for your records.
What happens next?
The Early Years SEND team will review your request and contact you if more information is needed. You’ll be informed of the outcome and next steps.
