Reablement support
Reablement is short-term care to help you regain skills and confidence after illness, injury, or a hospital stay. It focuses on what you can do for yourself, with support to build independence.
What does reablement involve?
- Support with daily tasks like washing, dressing, and preparing meals
- Therapy, equipment, or small home adaptations to make life easier
- Regular reviews to track progress
Key points
- Usually lasts up to six weeks
- Provided by HomeFirst or Independence at Home
- Free while you are actively taking part (up to six weeks)
- If you need ongoing care after reablement, normal charges will apply
Who is it for?
People who:
- Have had a hospital stay or illness
- Are at risk of going into hospital or a care home
- Already receive social care and need extra support
How is it arranged?
Reablement is usually arranged after a social care assessment or following a hospital stay. In many cases, an assessment will take place before you leave hospital to make sure the right support is in place when you return home.
