Sight loss

If you or someone you care for is experiencing sight loss, we can help through our Sensory Team and local services. Here’s how to get started:

Steps to Access Support

  1. See an optician or GP
    They’ll check your eyes and, if needed, refer you to an eye specialist (ophthalmologist).
  2. Confirm your diagnosis
    The ophthalmologist will explain your condition and treatment options.
  3. Ask about a Certificate of Vision Impairment (CVI)
    If your sight loss meets the criteria, the ophthalmologist will complete a CVI. This is the first step to registration.
  4. Referral to our Sensory Team
    We’ll assess your needs, give advice, and offer equipment or rehabilitation support. This includes help at home or in the community, tailored to your personal support plan.
  5. Create a personal action plan
    Based on your assessment, we’ll help you plan for daily living and staying independent.

Registering as Sight Impaired or Severely Sight Impaired

Registration is voluntary and confidential, but it can make life easier. It gives you proof of your sight loss and helps you access benefits and concessions.

How to Register

Once your ophthalmologist sends your Certificate of Vision Impairment (CVI) to us, we’ll get in touch to complete your registration. After this, we’ll send your registration card to you.
